Care Navigator – Out of Hours Service

 

Overview
We are recruiting a Care Navigator to support the smooth and efficient running of an Out of Hours service. This temp/bank role offers a mix of weekday evenings and weekend shifts. You will liaise with healthcare professionals, patients, and colleagues to ensure timely and high-quality care, helping the service run efficiently across multiple locations.

  • Mixed shifts: Weekday evenings 18:00–23:00; Saturday & Sunday 08:00–15:00 or 16:00–23:00
  • York based
  • £12.54 per hour

 

Responsibilities

  • Provide operational shift support to clinical teams in Out of Hours centres or remotely
  • Answer and make telephone calls in line with standard procedures
  • Manage patient queues in clinical systems to ensure efficient service delivery
  • Liaise with healthcare professionals regarding patient care and onward referrals
  • Coordinate face-to-face appointments and, where appropriate, home visits
  • Maintain accurate patient records and follow written procedures
  • Demonstrate excellent customer care, empathy, and a holistic approach
  • Report incidents or concerns affecting health and safety of staff or patients
  • Build and maintain professional relationships with colleagues and external stakeholders
  • Contribute to team activities and process improvements as directed by senior staff

 

Experience Required

  • Minimum Level 2 Maths & English (GCSE Grade C/4 or equivalent)
  • Ability to follow workplace policies and procedures accurately
  • Strong administrative, numeracy, and organisational skills
  • Excellent customer service and telephony skills
  • Ability to maintain confidentiality and work professionally
  • Excellent written and verbal communication skills
  • Strong organisational, prioritisation, and time management abilities
  • Able to work as part of a team and independently
  • Competent in Microsoft Office
  • Able to travel across various sites when required
